Players that have a permit or were nominated in the initial nominations forms that wish to play in a lower grade than they nominated/permitted to must receive an Existing Player Permit using the form below.

Player that are new to the competition and were not nominated or have a permit must receive a permit using the New Player Permit form.

Requests for review following completion of 3-game Provisional Permits must include player stats from the matches player, otherwise the permit request will automatically be denied.
